Shop directory · updated September 21, 2026

A supermarket, not a wellness brand.

Hi-Health Supermart Corp. runs a health supermarket out of 15207 N 75th St Ste 104 in Scottsdale, AZ. We buy shelf stock, we mark it up once, and we put the number on the label. Everything below is how the shop works, written out so you can read it before you spend anything.

What you will not find here is a story about a founder's health journey. We are a shop. We are good at knowing what is on the shelf, what is in it, when it arrived and when it expires, and we would rather be judged on that than on a photograph of a sunrise.

Customer service answers Monday to Saturday, 8am to 6pm Mountain at team@mychoicesmart.com. Anything that arrives after hours is picked up the next working day.

Who runs the shop

The shop is operated by HI-HEALTH SUPERMART CORP., EIN 86-0262033, from 15207 N 75th St Ste 104, Scottsdale, AZ 85260. That is the company on your receipt, the company on your card statement and the company that answers team@mychoicesmart.com. There is no second trading name and no separate online brand.

We are a retailer. We do not manufacture anything we sell, and we say so plainly on every shelf label by naming the maker. Where an item carries our own House Label name, the label still names the facility that made it, because a store label on a bottle is a purchasing decision, not a manufacturing one.

Why that distinction matters to you

A retailer that hides who made a product is asking you to trust the shop instead of the maker. We would rather you could check both. If you ever want the manufacturer and the lot for something you bought from us, email customer service with the receipt and we will send it the same working day.

How we price

One markup, the same percentage across the whole shop, applied to what we actually paid. It is published below. No department subsidises another, and no item is priced high in order to be discounted later.

What goes into a shelf price
ComponentWhat it is
Cost of goodsWhat we paid the supplier for that delivery, plus inbound freight.
Shop markupA flat percentage covering staff, rent, card fees and shrinkage. Identical on every item in the building.
Nothing elseNo anchor price, no recommended retail struck through, no was-and-now, no members-only tier.
  • Prices go down as well as up, and they follow the cost of the next delivery.
  • A price change applies from the day the new stock is booked in, never backwards to an order already placed.
  • Weekly specials are a genuine reduction against the shelf price, for a stated week, on a stated item.
Aisle 03

What we stock

Five departments. The shop is deliberately narrow, because a short list bought in depth is what keeps the markup low enough to publish.

Departments and what they cover
DepartmentWhat is in it
Everyday basicsMultivitamin and multimineral tablets and capsules for adults, including versions without added iron.
Single nutrientsIndividual vitamins and minerals, with the specific form printed on the label rather than just the element name.
OilsMarine and algal oils carrying their batch oxidation figures, plus plant-derived alternatives.
Plant extractsStandardised extracts only, where the marker compound and its percentage are stated.
Everyday digestionFibre and live cultures, with culture counts quoted at the end of shelf life rather than at manufacture.

Pack sizes and what each works out to per day are on the what we stock page.

Aisle 04

What we refuse

Suppliers pitch us every week. This is the list of what gets turned away, published so you can hold us to it.

  • Weight-loss products of any description.
  • Sports performance lines and anything sold on a physique.
  • Ingestible cosmetics, including anything promising an effect on skin, hair or nails.
  • Anything whose own packaging claims to treat, cure or prevent a condition.
  • Proprietary blends that list a total milligram figure without saying how it splits.
  • Anything built on one small study and a press release.

If you spot something on our shelves that belongs on this list, email customer service and quote the item. We treat that as a buying mistake and pull it.

Aisle 05

On every shelf label

The same six facts, in the same order, on every item in the shop, so two products can be compared without hunting.

Printed on every shelf label
FactWhy it is there
Maker and facilityWho physically made it and where. Not the brand owner, the plant.
Full Supplement FactsThe authoritative statement of contents. It overrides anything written elsewhere, including on this website.
Other ingredients in fullBinders, flow agents, capsule shell and colourings. Nobody with an allergy should have to ask.
Form of each nutrientThe salt or ester supplied, because two labels can name the same mineral and mean different things.
Lot and expiryRecorded against the sale so a recall reaches the right customers.
Test statusWhether a certificate of analysis exists for that lot, who issued it, and what it covers. Where none exists, the label says so.

Delivery and collection

Orders ship from Scottsdale, AZ by standard ground service. The figures below are what the carrier usually manages once the parcel leaves us, not a promise we are in a position to make on the carrier's behalf.

Usual time in transit after dispatch
DestinationUsual transitShipping
Arizona and the Southwest1 to 3 business daysIncluded
Rest of the continental United States3 to 6 business daysIncluded
Alaska, Hawaii and Puerto Rico5 to 10 business daysFlat rate, shown before you confirm
  • Add one business day for picking and packing. Orders in before 1pm Mountain on a business day go out the same afternoon.
  • Carriers do not count weekends or public holidays as business days, and neither do we when quoting the table above.
  • Tracking is emailed as soon as the carrier scans the parcel. Dispatch notices are never sent by text message.
  • If tracking goes quiet for two business days, email customer service. Chasing the carrier is our job.
Aisle 07

Returns

60 days from the day it was delivered, opened or unopened, with no restocking charge and no interview. Email team@mychoicesmart.com with your order number, or the delivery address if the number is lost, and we send the return instructions the same working day.

The refund goes back to the original payment method within 5 to 10 business days of the parcel reaching Scottsdale. Where the mistake was ours, because the wrong item was picked, the parcel arrived damaged, or the lot was short-dated on arrival, we pay the return shipping as well and replace the item at our cost. You do not have to argue that point; say what happened and it is dealt with.

Nothing here limits the rights you have under the law of your own state. Where the two differ, whichever is better for you applies.

Claims we will not make

Everything the shop sells is a dietary supplement. In United States law that means it is not a drug, and we will not describe it as one. Specifically, we do not claim, and we do not let a supplier's copy claim on our shelves, that anything we sell:

  • diagnoses, treats, cures or prevents any disease or condition;
  • replaces a prescribed medicine or the advice of a clinician;
  • produces a particular result in a particular number of days;
  • is clinically proven, when what exists is one small study;
  • is safe alongside your prescriptions, which only your own clinician can judge.

Required notice

These statements have not been evaluated by the Food and Drug Administration. Talk to a qualified healthcare professional before starting any supplement, particularly if you take prescription medication, are managing a health condition, or are pregnant or nursing. Nothing in the shop is intended for anyone under 18.

Text specials

Optional. Skipping this changes nothing about what you pay or how you are treated, and we would rather you skipped it than felt pushed into it.

If you want the weekly specials by text, add a US mobile number. Before you do, this is the whole of what you would be agreeing to.

  • Marketing messages only. Weekly specials and news about what has come into the shop.
  • Never order updates. Receipts, confirmations, dispatch notices and tracking always go to your email address.
  • Up to 4 messages per month; frequency varies. Message and data rates may apply.
  • US cell numbers only, 18 or over. A landline will not work, and neither will a number registered outside the US.
  • Text STOP back and they stop. Text HELP and you get our contact details. The removal form does the same job.
  • Your number stays with the shop. No mobile information is shared with third parties or affiliates for marketing or promotional purposes.
  • Not a condition of any purchase. See our Privacy policy and Terms of sale.
Ask us

Questions we get at the counter

Do I need an account to see prices?

No. Prices are on the shelf labels and on the what we stock page. A shop that hides prices behind a signup is asking you to buy the one thing you cannot inspect.

Will anything renew on its own?

No. Every order is a one-time purchase. No card is kept on file, there is no auto-ship, and there is no membership to cancel later.

Are the text messages my order updates?

No, and this is the distinction we are most careful about. Texts are marketing only: weekly specials and shop news. Receipts, confirmations, dispatch notices and tracking always go by email and never by text. Signing up is optional, separate from buying, and reversible at any time with STOP.

What happens to my mobile number?

It is stored against your record so we know who asked for the messages and when. It is not sold, not rented and not shared with third parties or affiliates for marketing or promotional purposes. Reply STOP, or use the removal form, and it is marked as removed.

Can you tell me what to take?

No, and we will not guess. We can read you a label, tell you the form of a nutrient, confirm a lot and expiry, or say whether a certificate of analysis exists. Whether any of it suits you is a question for your own clinician, who knows what else you take.

Can an order go to a different address?

Yes. Plenty of our orders go to a parent or to a workplace. Give the delivery address at the point of ordering; the billing name on the card does not have to match it.
